to your allocation summary. If your allocation summary specified a specific
item or a category, please use these titles to organize your answers. For
example, if you were allocated $40.00 for ingredients: under item purchased,
type “ingredients” and indicate the total spent on these ingredients. It is not
necessary for you to copy your receipt into this form. You will not be
reimbursed for amounts over the category specific allocation.
